CNA Registration Process
Step 1:
Fill out the registration form below. Register today to avoid the waitlist and save your spot immediately!
A Heart to Serve Representative will contact you shortly to discuss the enrollment process, important dates, payment options, etc.
Step 2:
Submit a copy of the following documents by Orientation Day (Dates TBD):
- Valid Government-Issued Photo ID Card
- Social Security Card
- Physical Exam and TB Test
- Immunization Records
- COVID-19 Vaccine
- High School Diploma or GED
- Live Scan Fingerprint
- CPR/BLS Certificate
Step 3:
IF you have not received your High School Diploma or GED, an entry exam is MANDATORY for enrollment. You must receive a passing score on the Wonderlic WBST Exam (basic Math and English questions), administered by a Heart to Serve representative.
Attend a MANDATORY orientation to be officially enrolled into the program (date and time TBA).

Frequently Asked Questions
The total tuition cost for CNA training is $1,500. This includes a $150 non-refundable registration fee due before the first day of class to guarantee a seat in the class. We also offer installment payment options.
The total tuition cost for CHHA training is $500.
Our CNA training sponsors are Skilled Nursing Facilities (SNF) that will facilitate Full Tuition Reimbursement. The sponsoring care health facility will reimburse your full tuition once you received your CNA license and guaranteed a job placement assistance. Rules and eligibility applies.
